Consent Form

• Every family must fill out one of these forms.
• You may list all your children on one form.
• If you register another child in the program, you need to fill out another consent form for that child.

Registration Forms

• Currently enrolled students need to fill out a form for each child in order to register for Fall classes (each form is specific for the type of class that you are requesting)
• If you want to register a sibling of currently enrolled student you need to fill out one of the forms below- (each form is specific for the type of class that you are requesting)
• New Students must fill out one form per child that you are looking to register (each form is specific for the type of class that you are requesting)
• Forms for each class can be found under the "Our Programs" tab.

Change Request Form

• Only for currently enrolled students who want to request a class change
• Click for Change Request Form

You must fill out the registration form AND the consent form to register for classes.
